Expense Reimbursement Application

  1. Reimbursement of Meeting/Event Expenses

    The City’s Culture and Engagement Committee supports meetings and events held by Shaker residents and organizations that are consistent with the Committee's mission and goals and that are open to the public.

    Please fill out the form below if you would like to seek reimbursement for such expenses. The Committee reviews and approves these requests at its monthly meetings, which are held via Zoom. You are welcome to participate in the meeting if you want to discuss your request but do not need to do so.

    Your request will be reviewed by the Culture and Engagement Committee at which time we will notify you of the results. If your request is approved, you will be required to submit copies of receipts for reimbursement.
